Vaccination Form

Time for vaccinations are near! It’s time to be ready for the 2023 season. Dr Smith will be holding a vaccination day at the fairgrounds on April 29th, 2023 from 10am-12pm. Be sure to note that the Strangles vaccination is listed as recommended by Purdue but in order to participate in LaPorte county fair it is REQUIRED. Below is what the vaccination form looks like.
